If yes, you can now go to the website www.storeopinion.ca to leave your feedback about your recent experience by participating in the Loblaw’s Store Customer Satisfaction Survey. Once you complete the survey, you’ll be entered into the Survey Contest for your chance to win a $5,000 cash prize or 1 million PC Plus points (if you’re eligible.)

- With multiple receipts, choose the most recent one to take the survey.
- Open to legal Canadian residents aged 18 or older without Loblaw staff and their immediate household members included.
- One entry per person per month.
- Participants must be a PC Plus member to earn the points.
- Winners are notified by phone or email within 4 business days after submitting the survey.
- Then, winners are required to complete and return some documents within 5 days after receiving them.
- Cash prizes will be mailed to winners within 4 to 6 weeks after receiving the documents and the points awarded as a credit to their PC Plus online accounts.
